The Role of Our Care Management Director
Every care journey begins with a comprehensive review by Assured Trust’s Care Management Director, who works closely with our Trust & Care Officers to understand the unique needs of each client. The Director then oversees the development and implementation of a personalized care plan – whether delivered by ACC’s skilled Care Managers or through trusted, verified community vendors.
Dedicated Care Managers
At the heart of ACC are its Care Managers, who ensure that clients receive consistent, quality care. Working hand-in-hand with our Trust & Care Officers, Care Managers provide a wide range of supports that enhance daily life, promote independence, and safeguard dignity.
Tailored Services to Meet Every Need
ACC offers three levels of care management services, designed to address a wide spectrum of needs:
- Level 1: Companion Care
In-home visits, wellness checks, light housekeeping, errands, organizing, or device/electronics support. - Level 2: Advocacy Support
Physician appointments, skilled nursing arrangements, relocation assistance, benefits review, and advocacy with schools, government agencies, or medical providers. - Level 3: Consultations & Care Plan Supports
Licensed professionals such as RNs, LSWs, therapists, or extraordinary support from Care Managers and Trust & Care Officers.
Community vendor services are also available.
